Job Description:Nurse DutiesProvide care to patients and residents based on their care plan outlined by their doctorAccurately monitor and record observations on patients' conditionsMaintain accurate records of residents care, financial procedures, and medication administrationIdentify care planning needsCarry out risk assessmentRecord patients' vital signs and medical informationReevaluate patient care plan as conditions changeConsult and coordinate with other healthcare team membersDirect or supervise less experienced nursing staffMonitor patients' diet and exercisePrepare patients and assist with treatments, surgery, or exams as neededManage stocks of suppliesEnsure storage and disposal of medication is carried out according to policyParticipate in clinical improvements and offer ideas on clinical and non-clinical policiesAssist with the investigation of complaints by patients or family membersMaintain continuing education and licensing requirementsReport any issues that arose during each shift Nurse Care Home DutiesAssume responsibility on each shift for the nursing care of allocated residents;Ensure a high standard in care for the elderly is maintained at all times;Lead the Health Care Assistant in providing high quality direct and indirect care;Build strong relationships with residents to encourage trust, and understand their needs;Evaluate resident care needs and in-conjunction with our multi-disciplinary team implement and evaluate individualised care plans that meet the residents needs;Form part of a dynamic environment where teamwork and  personal development are encouraged;Ensure all documentation relating to residents is accurately maintained in accordance with regulatory requirements;assessing and planning nursing care requirements;organising workloads;visiting patients at home;building relationships with, reassuring, and listening to and talking to patients;combating stigma and helping patients and their families manage it;administering medication;agreeing and reviewing care plans and monitoring progress;giving advice and arranging support for patients, relatives and carers;liaising with doctors, social workers and other professionals;assessing treatment success at case conferences and meetings;writing and updating patient records;encouraging patients to take part in therapeutic activities such as art and role play.
