Position Overview:

We are currently seeking a passionate and dedicated Primary Health Care Nurses to join our dynamic team. The successful candidate will play a crucial role in delivering comprehensive primary health care services, including health assessments, chronic disease management, immunizations, health education, and promoting preventive health measures across varied locations across rural and remote Victoria.

Key Responsibilities:

• Conducting health assessments and screenings.
• Managing and coordinating care plans for patients with chronic conditions.
• Administering vaccinations and medications as prescribed.
• Providing health education and counseling to patients and families.
• Collaborating with other healthcare professionals to ensure holistic patient care.
• Maintaining accurate patient records and documentation.

Qualifications and Requirements:

• Registered Nurse qualification with current AHPRA registration.
• Previous experience in primary health care or community health settings preferred.
• Excellent communication and interpersonal skills.
• Ability to work effectively within a multidisciplinary team.
• Strong organizational and time management skills.
