Job Description:Assess for aids and adaptations within the home environment to help increase independence and safety at home for both adults and children. The role involves a mix of community visits, working from the office base, and working from home. The post holder will work as a member of the multidisciplinary rehabilitation team.The post holder will support other teams as needed.The post holder will provide a comprehensive and high-quality OT and care management to patients with simple to complex needs within the intermediate care units and within the community.The post holder will be actively involved in team and service developments with the support of line manager.To develop strong working relationships within the multidisciplinary team.To provide supervision to band 5 OT(s), Rehabilitation Assistants and OT students whilst on placement in the service.To be actively involved in Continuing Professional Development.To be involved in caseload management with varied patients who have mobility, musculoskeletal, cognitive and neurological problems.Note:The team works closely with other services in the Centre for Independent Living.  Desk top assessment of Blue Badge applications to assess if eligibility criteria met for hidden or physical disabilities.Flexible working available. Needs to be HCPC registered.
