The Continuing Care Team provides community case management services for individuals over the age of 18, who are experiencing moderate to severe mental health issues and have difficulty accessing and maintaining social supports.  This is a multidisciplinary service. Case management provides the opportunity for the consumer to complete individual recovery and relapse prevention plans, receive evidence based therapeutic interventions, education regarding mental illness and contemporary treatment options, information and referral to other specialist organisations (government and non- government) as well as education, consultation and support for families and carers. The service is recovery oriented, person centred, family inclusive, collaborative and empowering; and operates on the premise that consumers can and do recover from mental illness and drug related disorders.

The Clinical Nurse (CN) role is to lead and deliver person centred nursing care at an advanced level based on the Nursing and Midwifery Board of Australia Registered Nurse Standards for Practice.

Key Responsibilities include:

  • Understand your broad responsibilities for safety and quality in healthcare, actively participate in continuous quality and safety improvement activities within the workplace and notify a relevant clinical or non-clinical responsible officer when concerns exist about workplace or patient safety.
  • Practice in accordance with code of ethics, professional standards and legislation affecting nursing practice by maintaining current knowledge and competence within scope of position.

The Health clinician will deliver Psychology, Social Work, Occupational Therapy clinical services as a member of a multidisciplinary health care team, to stakeholders/customers of Queensland Health, in accordance with the goals, policies and requirements of the Sunshine Coast Hospital and Health Service (SCHHS).

Key Responsibilities include:

  • Demonstrate competent mental health knowledge, expertise and skill in the provision of Psychology, Social Work, Occupational Therapy services commensurate with level of experience in accordance with prescribed professional and ethical standards, existing evidence-based practice and established clinical protocols. For complex or novel cases, you will seek direction from clinical seniors as appropriate.
  • Maintain appropriate documentation and patient records in line with Departmental, Unit and Health Service procedures, and contribute to the maintenance of accurate and timely information systems (statistics collection, quality data). Ensure that time spent across clinical and non-clinical work is in keeping with assigned requirements..

Remuneration value up to $128,706 p.a., comprising salary between $76,747 - $112,805 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP3) or Remuneration value up to $128,057 p.a., comprising salary between $104,816 - $112,236 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 6(1). (Permanent full time, 76 hrs p.f.). Applications will remain current for 12 months.
