Documentation of patient care is essential, along with monitoring, recording, and communicating patient condition appropriately to the care team, physician, patient, and family. You will assist patients with activities of daily living, provide patient and family education, and collaborate across disciplines to coordinate patient care. Additionally, you will evaluate the learning needs of patients and families and advocate for their physical and emotional well-being.
