Clinical Pharmacist lllPOSITION TYPE: Healthcare ProfessionalSPECIALTY: Clinical Pharmacy lll - PharmacyLOCATION: SHIFT: Abilene, TXJOB TYPE: Full-Time PermanentSalary Range : $54-$68/hrJob Summary:We are looking to hire a professional Clinical Pharmacist lll to be responsible for providing advanced clinical pharmacy services in a healthcare setting, including medication therapy management, drug information, and patient education. The role involves working collaboratively with healthcare providers to ensure safe, effective, and appropriate use of medications in patient care.Responsibilities:Provide clinical pharmacy services, including medication therapy management, drug information, and patient education, to patients and healthcare providers.Collaborate with physicians, nurses, and other healthcare professionals to optimize medication therapy for patients.Review and interpret patient medical histories, laboratory results, and other relevant information to determine appropriate medication therapy.Monitor and assess patient medication therapy for effectiveness and adverse effects, making recommendations as necessary to healthcare providers.Participate in medication safety initiatives, including identifying and reporting adverse drug events and medication errors.Provide education and training to healthcare providers and patients on medication-related topics.Develop and implement protocols and guidelines for medication use in specific patient populations or disease states.Serve as a drug information resource to healthcare providers, patients, and caregivers.Participate in formulary management, drug utilization review, and other pharmacy-related activities.Participate in research and quality improvement initiatives related to medication therapy.Job Benefits:Competitive salaryDirect Deposit401KComprehensive Health, Dental, and Vision benefitsEmployee discount programExcellent room for growth and advancementRequirements:Doctor of Pharmacy degree from an accredited school of pharmacy.Completion of a PGY-2 residency in a clinical pharmacy specialty or equivalent experience.Board certification in a clinical pharmacy specialty (e.g., BCPS, BCCCP, BCOP, etc.) preferred.Active pharmacy license within state of practice.Strong clinical knowledge and skills in medication therapy management, drug information, and patient education.Excellent communication and interpersonal skills.Ability to work collaboratively with healthcare providers as part of an interdisciplinary team.Strong analytical and problem-solving skills.Ability to prioritize and manage multiple tasks and projects effectively.
