Community Advanced Practitioner - Band 7

An exciting opportunity has arisen for a Community Advanced Practitioner to join the integrated Aberdeen City Health and Social Care Partnership. There are full time and part time positions available. Although this is a permanent position a secondment would be considered.

This role complements our innovative and inspiring community nursing model designed to facilitate transformational change in improving the health of our local population and delivering high-quality, integrated and tailored care.

The successful candidate will be responsible for the direct management of patients with complex needs in a locality and the teaching and supervision of nursing colleagues to develop in this area of practice. Close collaboration with primary care, secondary care and hospital at home colleagues will be vital to the success of the role.

Community experience is desirable and candidates must have broad based post-registration experience with objective evidence of ability to autonomously perform at Masters Level (SCQF 11), e.g. diagnosis using structured clinical history and appropriate examination. Applicants who are progressing through advancing practice qualifications will be considered.
