About you:
You are a qualified Clinical Nurse or Occupational Therapist or Physiotherapist or Social Worker or Psychologist with demonstrated high-level Aged Care knowledge (including dementia), with high-level knowledge, skills, and experience in conducting comprehensive assessments for complex client presentations, ability to provide leadership, precept staff, and contribute to professional allied health/nursing practice. You have demonstrated ability to contribute to and/or lead quality and service improvement activities, including participating in local service improvement initiatives.
Your opportunity:
To deliver high level clinical assessment completing comprehensive assessments of frail older people and their carers, recommending options and assistance to gain access to the most appropriate service to meet their care needs, including establishing eligibility for Commonwealth Programs of Aged Care Services as part of the FNQ ACAT and Memory Clinic Service.
