About us:

Our service is a hospital avoidance model designed to support the acutely unwell frail older person firstly to be cared for in the Residential Aged Care Facility (RACF) where possible, and secondly to provide rapid assessment and intervention for both RACF and community patients over the age of 65 years or 50 years for Aboriginal and Torres Strait Islanders within the Emergency Department.

About the role:

The Clinical Nurse utilises advanced clinical skills and leadership to ensure appropriate and timely care is provided to aged and frail patients in an optimum environment. The Clinical Nurse will:

  • Perform advanced gerontic clinical assessment of acutely unwell aged and frail patients.
  • Provide expert gerontic clinical guidance and act as a resource for the multidisciplinary teams within the emergency department and Residential Aged Care facilities.
  • Deliver an advanced standard of patient care including assessment, planning, implementation, and evaluation by using clinical nursing expertise including for patients with complex care needs.
  • Practice collaboratively and provide clinical leadership within the interdisciplinary health care team to promote the safety, security and personal integrity of individuals and groups.

About you:

To be successful in this role you must have:

  • A current registration through the Australian Health Practitioner Regulation Agency (AHPRA).
  • A relevant postgraduate certificate or post graduate diploma is an expectation of this position. Where the successful applicant does not currently hold this qualification, commitment to completion within an agreed timeframe is expected and will form part of the performance and development plan.
  • The ability to provide leadership, percept staff and contribute to professional nursing practice with confidence and integrity.
  • Competence in the application of clinical problem solving and effective coordination of person-centred care at an advanced level in varying environments.
  • The ability to demonstrate and apply well developed interpersonal and communication skills within a multi-disciplinary environment, including negotiation, consultation, and conflict resolution.

Salary information:

Total remuneration value of up to $10,671 p.m. is comprised of:

  • A hourly rate between $52.56 - $56.60 p.h.
  • 12.75% employer superannuation contribution
