Job Description

• Review bed utilization statistics and identify patients with issues that affect efficient patient flow. Conduct clinical case study by coordinating with the healthcare team and/or support staff on eligibility, social work, health status, clinical pathway, and others patient related care as applicable. Assess, facilitate and expedite diagnostic/treatment plans to ensure they are followed without any delay.
• Participate in making rounds in various wards/units as assign with the healthcare team. Organize group discussions and decision-making sessions when appropriate. Identify gaps in services or support structures. Collaborate with other healthcare providers in a multi-disciplinary and multi-dimensional manner.
• Collate and review patients’ clinical information including eligibility, movement/transfer, length of stay/delay and current health condition. Ensure that the doctors-in-charge write progress discharges including medical orders.
• Assist in developing and monitoring individualized treatment plans for physical, financial, and other constraints. Advise each patient on case management process including assessment, planning, implementation, evaluation, and transitioning. Ensure that individual patients understand their rights, responsibilities, and treatment plans. Refer patients to other practitioners who have skills and knowledge to best meet the patient’s needs.
• Facilitate and organize any clinical service delivery that is coordinated, timely, and appropriate for each patient’s optimal health care benefit, goal achievement, and maximal care. Ensure equitable access to healthcare services and use of healthcare resources that is ethically responsible. Provide assistance to the patients and their families, as and if needed.
• Develop and maintain clinical tools including investigation, data collection and other relevant information to complete assessments of patient needs. Review the case management discharge planning, clinical treatment process and daily huddle meetings.
• Ensure that all work is performed in line with all relevant policies and standards for all aspects of health and social services including mental and behavioral well-being, end-of-life comfort measures, and community supports.
• Prepare and present reports and correspondence. Maintain accurate and up-to-date patient records including accurate and timely charting and documentation of patient assessments, interventions, and outcomes.
• Act as an advocate for patients and their families.
• Perform other job related duties.
