The Role of Care Coordinator

We are looking for an experienced care coordinator who is qualified, reliable, and passionate. with planning and organising skills to meet both client and business needs. We are seeking an experienced, enthusiastic, and highly motivated Care Coordinator to coordinate, manage and administer the delivery of care services to our clients. Our clients are a mix of Private, Home Care Package and NDIS across all age groups. Care Coordinator must be willing but not limited to:

  • Delivering outcomes/goals for our clients based on their Home Care Plans/NDIS plan.
  • Collaborating with our clients to coordinate support and services to improve their quality of life.
  • Conducting client care assessments and care planning with a person-centered care approach in alignment with goals/outcomes and within budget constraints.
  • Understanding, managing, and working within government contractual requirements.
  • Supervision and management of Care Staff.
  • Schedule appointments and in-home services to appropriate staff based on discipline, availability and geographical requirements with our unique carer matching at the forefront of decisions.
  • Coordinate the provision of quality person centered care according to the client's needs, service/care plans and available funding in accordance with budgets.
  • Building relationships with local stakeholders
  • Positive attitude with the ability to work in an ever-changing environment.

Qualifications

To be successful in this role, we expect you to have:

  • EN/RN in community nursing 1-2 years with current AHPRA registration
  • And or Cert III/Cert IV (Aged/Disability Care) minimum 4years experience
  • Experience within the aged care/disability/mental health in community setting
  • Understanding of Aged care & Disability standards and consumer directed care.
  • First Aid, current CPR and National police check
  • WWC (or willingness to obtain)
  • Driver’s license - Australia
  • High level of computer literacy
  • Understanding of clinical governance and continuous improvement
  • Possess a 'can-do' positive attitude and work well in a team environment.
Preferred Skills
  • An outgoing personality with high energy levels, good communication skills and personable telephone manner
  • Ability to problem solve and think quickly under pressure.
  • Ability to multitask with a strong attention to detail while keeping calm and focused.
  • Excellent networking, consultation skills to develop new referral sources and relationships with appropriate external organizations.
  • Excellent customer service skills with exceptional verbal and written communication skills.
  • Sound knowledge and understanding of home care services and guidelines.
  • knowledgeable in wound care, catheter care, peg feeding etc.
  • Sound knowledge and understanding of NDIS services and guidelines.
  • Previous experience with rostering is highly desirable.
