We have a growing Clientele and are requiring a Registered Nurse to join our current Nurse Care Co-ordinator team....

This position can be flexible in working hours to cater for Family needs....(i.e. 9am-3pm X 5 days a week)

Company Background:  Right at Home Southwest Victoria is a local home care provider which started in 2022 and is quickly gaining a respected reputation. We provide a wide range of care services all designed to help our clients stay in their homes. Our home care services include companion, personal, nursing, respite, transportation, aged, disability, transitional, 24 hour, and palliative care.

Purpose of Position: This role is customer focused and involves consulting with families, preparing in home care assessments, and drafting care plans to best meet client requirements. This position is responsible for care co-ordination duties, this position will help develop relationships with Clients and their families, hospitals, clinics and care facilities with a view of delivering care services to meet the market.

Duties include:

  • Dealing with a range of seniors and frail elderly customers and their families. This role will handle anything from home care, information requests, feedback, general enquiries to complaints
  • Ensuring all client contact is managed in an empathetic, friendly, interested and patient manner to quickly build rapport, trust and confidence in Right at Home.
  • Providing client and family education on the nature of services that are available and the effect and benefit of the care plan.
  • Preparing in-home care assessments and drafting care plans to best meet client requirements
  • Managing government funded cases as well as private pay clients
  • Training, and managing the performance of the care team
  • Participating in community activities or committees as requested
  • Ensuring highest quality service is consistently delivered
  • Assisting management with business development opportunities across a range of referral sources
  • Operate and maintain care planning and scheduling technology
  • Ensure all documentation is accurately and comprehensively completed in Right at Home databases and software programs in accordance with organisation policies and procedures and legislative frameworks.
Qualifications
  • National police check
  • Current First Aid and CPR
  • Reliable source of transportation required
  • Current AHPRA registration & Professional Indemnity Insurance
  • Working with Children Check Certificate (Preferred)
