About you:

In your new role you will…

  • Demonstrate an ability to provide high level diabetes clinical services, leadership, and coordination within a multidisciplinary environment.
  • Demonstrate ability to lead program, project, and quality management activities including the ability to prepare relevant reports, strategic responses and documents for change management processes and to advance program outcomes.
  • Demonstrate knowledge and experience in the analysis and synthesis of health-related data to inform and support health service planning processes and priorities.
  • Demonstrate advanced knowledge and experience of contemporary evidence-based practice and clinical service delivery models in diabetes management and ability to apply this knowledge strategically to achieve a redesign and innovations solutions to complex health system issues.

About your opportunity:

In this position you will provide advanced clinical expertise and project/program management skills to develop, implement, and coordinate an integrated multidisciplinary diabetes service for CHHHS. The program coordinator role will work towards ensuring CHHHS has a high quality, safe and sustainable diabetes service that meets the health care needs of people living with diabetes.

About your role:

  • Provide high quality advice in the development, implementation, and management of the Connected Community Pathways Program, in collaboration with key stakeholders and the governance committees employing effective change management strategies to facilitate achievement of program KPI's.
  • Lead with influence at various levels to facilitate the operation, organisation and cultural change associated with the investment in and implementation of innovative models of diabetes care.
  • Manage and maintain strategic partnerships with a range of internal and external stakeholders to ensure the delivery of project objectives. Consultation, negotiation and advocacy with other service providers and managers through networking and collaborative partnerships is a key component of this role.
