Your opportunity: 

• The Clinical Nurse (Discharge Planner) is a clinical specialist, accountable at an advanced practice level for the coordination of community support options, by utilisation of quality and cost-effective resources and delivering discharge care that is driven by the patient's needs. The Clinical Nurse (Discharge Planner) demonstrates sound knowledge of contemporary nursing practice and theory and supports the Nurse Unit Manager and other members of the Integrated Care Services team with the timely transitions of care through the patient journey and the provision of clinical leadership, advice and advocacy to a multidisciplinary team.

Permanent Part Time, 36 hrs p.f. position along with several Temporary Full Time, 76 hrs p.f. for periods of leave relief until March 2025 positions available.
