Job Description:Band 6 Looked After Childrens Nurse:Job Responsibilities:To provide ongoing advice and support to residential care staff with regards to the health needs of LAC and Care leavers.To consult with and evaluate LAC and Care Leavers experiences of health services, and develop services that are responsive to their needs.Work within the professional boundaries of confidentiality and consent, balancing the need to share information and safeguard LAC and Care Leavers.Ensure all LAC and Care Leavers are provided with a comprehensive health summary upon leaving care.To maintain full, accurate and contemporaneous electronic health records in line with the Trust Record Management Policy.To be responsible for undertaking and coordinating health reviews for LAC, using agreed health assessment tools in order to improve LAC health outcomes & access.Working together with CYP to explore the best ways to engage them and meet their health needs, provided within the context of the core values of the Trust for compassionate care and respect for all.Support the effective implementation of transition protocols for LAC and those leaving care, moving from childhood through to adulthood.Provide telephone/helpline advice to children and young people, those with parental responsibility, foster carers and residential social workers; acknowledging consent issues appropriate within local and national legislation or guidance.Provide drop-in sessions to children and young people as appropriate & contribute to multi professional support days.Provide a liaison role that offers sign posting, child specific liaison and support to access other services, such as Child and Adolescent Mental Health Services, CAMHS-LAC team, Substance abuse, Dental and Integrated Sexual Health Services (ISHS) and the CSE Nurse Specialist.Ensure LAC CYP are given opportunities to have time alone to express any concerns or personal issues to support their health, safeguarding & social care needs; particularly during health assessments.To play a key role in increasing the immunisation uptake for Looked after Children.To travel to other boroughs as required to ensure effective assessment and implementation of health care plans.To use RiO in case management of children and to maintain an overview of the caseload.
