Position Summary

Looking for an experienced nursing leader for the Wrinch Memorial Hospital. If you are passionate about providing effective and efficient delivery of hospital services, please look into this position.

In accordance with the established vision and values of the organization, the LPN Home Support Team Leader/Primary Care Nurse, coordinates Home Support Services by supervising Community Health Workers, overseeing the implementation of client care plans, providing training and evaluation of personal care techniques, provides advocacy services, and also functions as a member of the interprofessional team and applies best practice to provide competent, safe, and ethical care for people and their families in settings such as physician practices, homes, schools, and communities. The Home Support Team Leader/Primary Care Licensed Practical Nurse performs assessments, plans, and implements and provides personal and nursing care to people. The LPN administers oral, subcutaneous and intramuscular medications in accordance with pharmaceutical and organizational policies and procedures. The scope or extent of the skills that the LPN performs varies according to the predictability of the outcome for a given population. The LPN operates in accordance with the competency guidelines and full scope of practice within the Standards of Practice as outlined by BC College of Nurses and Midwives (BCCNM) and according to organizational policies, procedures, standards and unit specific protocols.

Shift Rotation/Hours of work: Monday to Friday, Days - 08:00 to 16:00

Wrinch Memorial Hospital Teaching is a time honored tradition at Wrinch Memorial hospital. There is an ongoing rotation of Medical Students, Nurse Practitioner Students and Medical Residents. It serves a population of 8,000 with:
• 9 Complex Care beds
• 1 Respite bed
• 10 Acute Care beds
• Pharmacy
• Dental Clinic
• Laboratory
• Doctor’s Outpatient Clinic
• 1 secure psychiatric observation room
• Medical Imaging including ultrasound

Other health and community care services include:
• Public Health
• First Nation’s Health Authority encompassing Gitxsan Health
• Mental Health and Addictions
• Skeena Place Assisted Living Complex
• Home and Community Care support services
• Diabetic education
• Dietician services
• Visiting specialists including Podiatry, Pediatrician, Internal Medicine, General Surgery, Rheumatology, OB/GYN, Cardiology and Geriatric Assessment Team
• Pre and Post-Natal outreach programs

The communities that make up what is collectively known as the Upper Skeena, The Hazeltons , are situated on the unceded traditional territory of the Gitxsan Nation. The Gitxsan people have lived on these lands for 10,000+ years and along with the Wet'suwet'en Nation to the east, established the precedent setting Delgamuukw court decision in 1997.

Some of the communities included in the Upper Skeena are the incorporated municipalities of Hazelton and New Hazelton, unincorporated settlements of South Hazelton, Two Mile, Suskwa Valley, Miskinish (Cedarvale), Kitwanga Valley, Andimal and the Kispiox Valley, and the 7 Indigenous villages of Gitanyow, Gitwangak, Gitsegukla, Hagwilget, Gitanmaax, Sik-e-dakh, and Anspayaxw. Qualifications

• Registration with BC College of Nurses and Midwives as a practicing LPN registrant.
• Two (2) years of recent, related experience including one (1) year in a supervisory role; or an equivalent combination of training and experience.
• Current valid BC Driver’s License.

• Demonstrated knowledge and skills within a specific clinical specialty or patient population dependent on the nature of the caseload (e.g. acute care, gerontology, palliative, etc.).
• Ability to communicate effectively, both verbally and in writing.

• Ability to observe and recognize changes in clients.
• Ability to plan, organize and prioritize; and ability to implement a plan of care.
• Ability to operate related equipment including proficiency in relevant computer applications.
• Physical ability to carry out the duties of the position.
• Use of own vehicle.
