Position Summary

As a valued member of the 0utpatient paediatric Team the Registered Nurse has multiple accountabilities to support all outpatient paediatric clinic service needs. The primary role as a multidisciplinary team member for the Paediatric Diabetes Education Centre (PDEC) and the Outpatient Paediatric Services Team. The Registered Nurse Diabetes Educator is responsible for providing diabetes education for the prevention and management of diabetes, as per best practice guidelines. Clients include patients and families referred to the PDEC with prediabetes, type 1 and type 2 diabetes and provides support to all of the outpatient clinic services within the Paediatric Service Model as required.
The Diabetes Nurse Educator acts as a consultant for PDEC and participates in the overall coordination and planning of the program, as per the mission, goals and objectives of Orillia Soldiers’ Memorial Hospital (OSMH). The Educator will also act as a resource for regional community partners and Inpatient Paediatric Services for all patients and families diagnosed with diabetes.
The Diabetes Nurse Educator will support Outpatient Paediatric Clinic services as required and outlined by Program Manager.
The Paediatrics Diabetes Centre will also work with a targeted population of First Nations individuals up to an including age eighteen (18).
PRIMARY RESPONSIBILITIES:
1. Patient Care
• Assisting patients achieve their optimal level of health.
• Providing support to patients in accepting, understanding and living with diabetes.
• Completing a patient assessment and meet all Ontario Diabetes data requirements. Patient needs will be identified based on laboratory, physical, nutritional and psycho/social factors. Assisting patients to develop self-management goals to promote empowerment in diabetes care.
• Developing and implementing client/family centered diabetes education and management plans based on patient’s self-management goals.
• Providing education relevant to the patient’s individual needs, including disease process, prevention and care of complications or comorbidities, including self-monitoring blood glucose, sick day management, medications and insulin therapy. The approach includes one-on-one, family and group education. Current educational and behavioral models/theories are utilized (i.e. Stages of Change, empowerment, self-efficacy, etc).
• Providing ongoing evaluation of patient care based on laboratory, physical, nutritional and psycho/social factors. Follow-up will include individual, group and/or telephone, as required.
• Initiating patients to insulin therapy (daily, multiple daily injections (MDI) or continuous subcutaneous insulin infusion (CSII)), based on physician orders. This includes insulin self-administration, dose adjustment and teaching patients self-management practices.
• Documenting all patient related care in required data base and sending physician reports as per PDEC policy and procedure.
• Collaborating with physicians, as required, to evaluate patient progress/needs.
• Acting as a resource person for diabetes education, based on collaboration with allied health professionals and community partners, with recommended referrals as required.
• Providing diabetes education according to the practice standards of the College of Nurses of Ontario.
• Partnering with Family Care Providers, physician, NP and family to ensure optimal health and well-being is achieved
2. Program Co-ordination
• Participating in Program Planning Committee meetings.
• Participating in the planning, implementation and evaluation of the PDEC, to include the PDEC program, health promotion activities and education (ie Health Professional Day).
• Participating in the development, evaluation and revision of PDEC policies and procedures to ensure up to date in all practices and policies.
• Participating in OSMH workload measurement.
• Collaborating/communicating with physicians/NPs and other health professionals to ensure consistency and continuity of program delivery.
• Coordinating inpatient paediatric education for clients and families upon discharge.
• Promoting awareness of PDEC services and programs within North Simcoe Muskoka Ontario Heath Team (LHIN).
• Provides support as a RN to other outpatient clinics as required by team and/or Manager.
• Assuming responsibilities delegated by the Manager.
• Participating in OSMH committees/meetings as delegated by the Manager.
3. Professional Development
• Maintaining Diabetes Educator Certification and license with the College of Nurses of Ontario.
• Attending appropriate workshops, professional conferences, in-services and participating in e-learning, journal and literature reviews as required to update knowledge and skills pertaining to diabetes education and management, to include, clinical practice guidelines, current nursing practice, educational and behavioural models/theories, insulin delivery systems including insulin pumps, glucose monitors and continuous glucose monitoring systems.
• Identifying areas that require additional in-service education in the Nurse Educators own practice and access programs aimed to assist in developing said practices.
• Participating in clinical projects/studies and committees, both specific and hospital wide, for the improvement of patient care.
• Acting as a liaison with other health professionals within OSMH e.g. inpatient paediatrics and community partners.
• Maintaining proficiency in computer skills.

Qualifications

Education:
• Current general class registration with the College of Nurses of Ontario (CNO) required
• Bachelor’s Degree in Nursing preferred.
• Current Certificate of Competence in the Province of Ontario required.
• Current Basic Cardiac Life Support (BCLS) required.
• Paediatric Advanced Life Support (PALS) required
• Certified Diabetes Educator required.
• Membership in the Diabetes Educators Section of the Canadian Diabetes Association an asset.
Experience:
• Minimum three (3) to five (5) years Paediatric nursing experience required and evidence of Paediatric competency required.
• Experience with paediatrics education, presentation and public speaking essential
• Experience with paediatric diabetes education to patients, families is required
• Experience with teaching and leading program change is required
• Experience with Leadership is required
Competencies:
• Demonstrated knowledge of chronic disease prevention and management of paediatric diabetes.
• Knowledge of insulin therapy, to include MDI and CSII and others. Will be required to obtain and maintain insulin adjustment competency and medication dispensing certification requirements.
• Demonstrated computer literacy with some advanced computer skills.
• Demonstrated strong communication skills in English, both written and verbal.
• Demonstrated ability to organize, prioritize and multi task efficiently.
• Demonstrated ability to maintain confidentiality and work in a highly professional manner.
• Demonstrated ability to educate, facilitate and present information clearly and precisely.
• Demonstrated ability to work both independently and in a team environment.
• Demonstrate ability to lead a team for optimal success in patient goal attainment.
PATIENT SAFETY:
Participates in and complies with hospital safety and infection control programs.
• Reports unsafe conditions.
• Assists in the reporting and investigation of incidents.
• Reviews related policies and procedures on a regular basis.
• Maintains patient and Hospital confidentiality.
• Responds appropriately to codes.
• Participates in quality activities, as required.
• Ensure compliance with the Hospital’s patient safety, staff safety policies and appropriate legislation.
• Promotes patient safety and staff safety in the workplace.
