About you:

In your new role you will…

  • Demonstrate high level clinical knowledge and skill in Pharmacy practice as well as recognised expertise obtained through significant professional development activities, post graduate education or formal qualification(s).

About your opportunity:

In this position you will deliver a specialised Clinical Pharmacy Service within the Renal Pharmacy team and contribute to practice and service development, innovation and activities that improve medication management for patients accessing services in accordance with organisational values, goals, professional, legislative and ethical standards.

About your role:

  • Develop, implement, monitor and review departmental and organisation's quality standards, work health and safety policies, procedures, programs, clinical practice and clinical governance in the CHHHS Pharmacy and Renal Department.
  • Provide independent high level clinical pharmacy services (face to face, phone or telehealth) of a complex and critical nature with significant scope to CHHHS and community health clinics including all Renal services (Kidney Supportive Care, Transplant, Dialysis and Chronic Kidney Disease).
  • Provide advice and assistance to multi-disciplinary health professional staff about pharmacy services, policies and procedures, and promote quality use of medicines, medication safety principles and the role of the pharmacist within the CHHHS Renal Department.
