Job Description:Take responsibility for the management of a complex caseload, which includes, specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans for CYP with learning disabilities.Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.Assess patient's individual care needs, develop, implement and evaluate programmes of care.To work as part of the Children's Community Learning Disability Team assessing the health and emotional wellbeing needs of children and young people with moderate to severe learning disabilities and responding appropriately.To complete specialist nursing assessment, development, implementation and evaluation of interventions to promote the health and wellbeing of children, young people and families affected by learning disabilities and or complex physical and mental health needs.To work as an integral member of the multi disability team maintaining responsibility for a designated caseload acting with autonomy as appropriate.To positively represent the needs and rights of children and young people with learning disabilities, promoting equality and access to NHS and any other service.
