About the role

  • The position delivers high quality; evidence based Social Work clinical services and leadership in the assessment and treatment of consumers with complex mental health issues, their families, carers and relevant stakeholders, in an acute and post-acute setting. This position operates as a member of a multidisciplinary mental health team with a focus on planning and providing interventions within a therapeutic recovery framework.
  • This role will deliver leadership in the co-ordination, delivery and evaluation of the Transitional Care clinical model within TPCH Acute Care Team.

In terms of technical knowledge, skills and experience and within the context of the responsibilities described above under Key Accountabilities, the ideal applicant will be someone who can demonstrate the following:

  • High level Social Work clinical skills within an acute care framework including assessment, formulation and intervention capabilities that support effective crisis management within a recovery orientated and trauma informed approach.
  • Ability to develop and maintain collaborative partnerships with identified key stakeholders including Emergency Services, NGO's and other organisations via formal partnership agreements, memoranda of understanding or protocols and regular liaison.
  • Demonstrated experience in the identification, development and implementation of quality and service improvement activities.
  • Demonstrated ability to provide clinical practice supervision to less experienced staff members and education to students.
  • Demonstrated high level interpersonal savvy to work effectively within a multidisciplinary team in an acute care setting.
