The Community Palliative Care Team align with all the NWHHS values and provide high quality care for all individuals in the region. 

Permanent full time.

  • Provide a culturally responsive primary healthcare service to Aboriginal and Torres Strait Islander and their families.
  • As part of a multi-disciplinary team, provide expert culturally specific knowledge to clinicians, staff, service providers and local communities.
  • Assist clients by acting in a liaison and support role to ensure that they understand and can actively participate in their own health care.
  • Demonstrated local cultural knowledge and exercise cultural respect and safety at the individual and family levels and provide cultural awareness training to non-local health staff.

Base salary rates between $2,722.20 and $2,910.80 p.f. (HW4)

Requirements

Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care is mandatory for appointment at the HW4 level.

Under s25 of the Anti-Discrimination Act 1991, there is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander Community.

Travel to work at various facilities including remote sites within the NWHHS, by air or road travel, including day trips and/or extended trips over several days is a requirement of this role and a C Class Motor Vehicle Licence is required for operation of fleet vehicles.

Pre-employment screening: working with children (Blue Card) and criminal history checks.

This is a Vaccine Preventable Diseases (VPD) risk role and may require evidence of vaccination or immunity to: Hepatitis B, Measles, Mumps, Rubella, Varicella, Pertussis, Tuberculosis and COVID-19.

A 3-month period of probation may apply.
