About the Role

The Mental Health Triage clinician role is pivotal to the Wembley Road community facility for the first point in contact for people seeking assistance with:

  • Mental Health Triage and assessment for walk in consumers.
  • Responsible for planning and coordination of care following assessment or discussion and with the medical team.
  • Responsible for daily duty intake Monday – Friday 0830-1700pm (last assessment 1600pm).
  • Referral pathways to internal and external teams following ad hoc review of referrals held daily.
  • Assist the team with consumers that may be on the duty intake register for immediate tasks for follow up.
  • Receive verbal handovers from other teams/services regarding Transfer of Care and liaise with Team Leader for action.
  • Medication assistance for consumers that attend the clinic for Long Acting Injectables (LAI's).
  • Provide immediate assistance with applications for an Examination Authority under Mental Health Act 2016.
  • The Clinical Nurse plays a pivotal role in providing in person support for the attainment of consumer goals including the development of independent living skills.
