About the role
The purpose of this role within the Community and Oral Health Directorate is to apply high level physiotherapy skills and knowledge in the assessment and treatment of clients in the local geographical population in order to promote, maintain and improve their health, functional ability and quality of life.
The Community Transition Care Program operates with extended opening hours – 0800 hours to 1700 hours seven days per week including public holidays. Only nursing is required on weekends and public holidays. Further extension of operating hours may occur based on client and hospital and health service need.
