About the role

Permanent part-time role, 48 hrs p.f. 

This position works with the Aged, Frail and Palliative Care team, and is committed to enhancing and enabling positive patient experience and outcomes by fostering support, collaboration, and partnership with patient, families/carers and other health service providers at Mount Isa Hospital and the outlying facilities of the North West HHS.

Palliative Care Service provides community and in-home palliative care to patients and their families of the North West HHS.

The Palliative Care Service works in partnership with:

  • Specialist Palliative Care and Rural Telehealth (SPaRTa) who support clinicians in rural and remotes ares to provide palliative and end-of-life care for patients and their families.
  • BlueCare – Community Based Palliative Care Program – entry to the program is via referral from the North West Palliative Care Clinical Nurse Consultant.
  • PallConsult – is a 24/7 phone advice service for clinicians who wish to speak to a palliative care doctor or nurse for immediate service. PallConsult also provide education and teaching.
  • General Practitioners

Requirements:

  • There is an expectation that applicants will possess or be working towards completion of an Advance Care Planning Australia online course.
  • Mandatory requirements are a relevant tertiary degree qualification and appropriate registration/licences.
  • Travel may be required. C Class Motor Vehicle Licence required to operate vehicles.
  • This is a VPD risk role and may require evidence of vaccination or immunity to: Hepatitis A/B, Measles, Mumps, Rubella, Varicella, Pertussis, Tuberculosis.
  • Aged Care pre-employment screening will be conducted.
  • A 6-month period of probation may apply.
