About The Role

The Nurse Practitioner Candidate/Nurse Practitioner supports our current model of care where exacerbations of chronic conditions are minimised, prevented and/or managed in the community through responsive, integrated person-centred care. This role will work across the Emergency Department and Community to divert people from the Emergency Department and reduce inpatient stay. People with specialist knowledge in Chronic Heart Failure and Chronic Respiratory Disease are encouraged to apply. The program utilises a combination of time spent within the Emergency Department, clinics and an assertive outreach model that includes home visits as appropriate to clients residing within the Monash Health catchment.

This role is integral in the assessment and management of client's wellbeing and operates from a person-centred care framework and aligns with the social model of health. 

About You:   

  • You are a highly motivated, dynamic Nurse Practitioner Candidate/Nurse Practitioner with a passion for chronic disease who will provide support to the most vulnerable people in our Community with Chronic Heart Failure, Chronic Respiratory Disease.
  • Proven ability to work both independently and collaboratively. 
  • You readily build strong relationships with stakeholders in the delivery of client centred care. 
  • You have strong written and verbal communication skills.

What you need: 

  • Demonstrated recent experience as a Nurse Practitioner Candidate/Nurse Practitioner/advanced practice nurse
  • Current AHPRA registration 
  • Endorsement as a Nurse Practitioner/enrolled in/willing to undertake Master of Nursing Nurse Practitioner
  • Valid Driver's licence and good driving history
  • Previous Community Nursing experience
