The Role

An opportunity has become available for two Clinical Nurse - Clinical Coordinators (Subacute and Non-Acute Episodes of Care) to join Gold Coast Health (GCH) where the focus of our 12 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.

You will

  • Apply high level knowledge and skills in enacting Subacute and Non-Acute Patients (SNAP) episodes of care changes in acute inpatient units
  • Exercise independent professional judgement in decision-making and clinical management, handling an increasingly complex and varied caseload
  • Contribute to multidisciplinary team management of referrals received via the SNAP referral pathway
  • Apply high level evidence based practice to contribute to the knowledge development and competence of hospital inpatient units

About you

To be successful in this position you will require:

  • A Bachelor of Nursing or equivalent
  • A graduate certificate or greater within a relevant specialty area is highly desirable
  • AHPRA Registration
  • Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
  • Tuberculosis screening, to identify if the tuberculosis vaccination may be required
  • Declaration of serious discipline history must be disclosed
  • Criminal History Screening will be required for new employees (GCH will cover cost)

Salary

  • Permanent full time (two positions)
  • Base salary ranging from $124 435 - $ 133 901 per annum

About Us

This role works within the Rehabilitation Response Team (RRT) service, which is a specialised, multi-disciplinary service that follows a shared care model to provide rehabilitation services for inpatients in an acute ward setting (inreach rehabilitation). Within this, the RRT provides assessment and management of all referrals for inreach and inpatient rehabilitation services, the Complex Management Unit and rehabilitation episodes of care via clinical intent.
