Registered Nurse – Integrated Comprehensive Care

Looking for an RN to work in our ICC program

Hours of Work: Full Time Days

POSITION SUMMARY:

St. Joseph’s Home Care is dedicated to providing quality and compassionate care to individuals in their homes and/or alternative settings through visiting, shift or the Integrated Comprehensive Care Programs. ICCP is an innovative model of care that supports the seamless transition of patients from the hospital to the community. Registered Nurses enhance the life of their clients by providing a range of care to clients with predictable outcomes, within the framework of SJHC’s policies and procedures and the standards of practice prescribed by the College of Nurses of Ontario. Through their work, Registered Nurses contribute to the healing and independence of their clients and promote the continual improvement of the overall health of the community.

RESPONSIBILITIES:

  • Utilize the nursing process and standards of care (College of Nurses of Ontario) to work independently in the community and provide the highest quality care through completion of appropriate client assessments and implementation of the plan of care.
  • Coordinate effective and safe care for clients; ensure appropriate caseload management, and ongoing monitoring and evaluation of care.
  • Provide effective nursing care primarily to clients within the Integrated Comprehensive Care Program and to the clients of the Hamilton Visiting Program as required.
  • Collaborate respectfully with the client, family members, other team members, Physicians and the LHIN (where appropriate), advocating for the client/caregivers by interpreting information, supporting decisions and facilitating communications. Utilize technology in order to effectively document and communicate with other team members and Physicians.
  • Apply current knowledge and research to practice. Maintain current competencies, participate in continuing professional development opportunities and obtain advanced or specialized nursing qualification/skills to meet client care needs. Carry out self-appraisals in accordance with the College of Nurses of Ontario Standards of Practice and Self-evaluation and the SJHC appraisal process.

QUALIFICATIONS:

  • Current RN registration with the College of Nurses of Ontario with no restrictions on licence or hearings pending, AND
  • Registered Nursing Diploma from an accredited college program with evidence of IV medication administration as part of the basic educational curriculum OR
  • Registered Nursing Diploma from an accredited college program and IV medication administration certificate from an accredited college program if this content was not included in the basic diploma educational curriculum OR
  • Registered Nursing Certificate from an accredited college program, a medication administration certificate and an IV medication administration certificate from an accredited college program
  • Current registration with the College of Nurses of Ontario
  • Basic Cardiac Life Support (BCLS)
  • Demonstrated experience working with the following ICCP patient groups: congestive heart failure, Chronic Obstructive Pulmonary Disease, Thoracic surgery, peritoneal dialysis trained, elective hip/knee replacement.(Chronic disease management and peritoneal dialysis experience is an asset)
  • Ability to function to the full scope of practice as per the College of Nurses of Ontario
  • PPS
  • ESAS
  • Can effectively use software for client documents, word processing and communication
  • Committed to customer service

  • Demonstrated commitment to professional development and continuing education
  • Good work performance and attendance record are essential
  • Certificate of Registration from Ontario College of Nurses with proof of payment for current year
  • Demonstrates the following competencies: Service and Mission Focus, Achievement, Teamwork, Learning and Innovation, Communication, Conflict Resolution and Resilience.
  • Valid Driver’s License. Proof of $1M automobile liability insurance. Able to drive locally and on the highway.

This is a community nursing position with requires you to driving to patients homes.
Our Integrated Comprehensive Care Program (ICCP) services Hamilton, Niagara Falls, St. Catherine, Guelph and Brant areas. Are you able and willing to drive in all areas?

Wages- SJHC is a unionized work environment and we have Collective Agreement in place for both RNs and RPNs.
RNS are unionized with the ONA group and RPNs are unionized with the LIUNA Local 3000
RN (ONA) wage band has 11 levels. $29.59-$43.81
RPN (LIUNA) wage band have 5 levels $25.94-$28.12
Each level is equivalent to 1500 hours worked with a previous employer.
Driving time is paid as part of your day. SJHC currently pays .52 cent per km you drive in addition to hourly wage.
