The Clinical Nurse Consultant - RACF Liaison Nurse role is an exciting new initiative to assist patients and their families navigate the pathways from Royal Hobart Hospital (RHH) to Residential Aged Care Facility (RACF) in a collaborative and multidisciplinary framework.

You'll be based in the Integrated Operations Centre:

• The Integrated Operations Centre (IOC) is located on the Ground Floor, C Block, RHH. The IOC is a coordination hub that ensures the right person receives the right care in the most appropriate setting.
• The IOC at RHH has integrated services that work together and provide excellence in health care to ensure a smooth patient journey across the care continuum, connecting our patients to the right level of care is our priority.

Details of Appointment

Fixed term, full time or part time, shift-worker positions working up to 76 hours per fortnight, commencing as soon as possible until December 2026.

*notwithstanding hours may be negotiated with the successful applicant

• Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time and part time vacancies.

$111,506 - $118,336 per annum. Our Employer 11% superannuation contribution is on top of this amount.

You’ll have access to salary package a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible.

In addition, the following allowances calculated on the salaried incremental point may be available:

• Post Graduate Allowance of up to 7.5% based on qualification type
• Professional Development Allowance of up to $500 per annum
• Salary range is in accordance with the Nurses and Midwives (Tasmanian State Service) Agreement 2023

Successful applicants will be required to meet the essential criteria:

• Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse

Applicants should note the following criteria are desirable:

• Holds or is working towards a relevant post graduate tertiary qualification and/or recent extensive nursing experience relevant to the role.

The Head of the State Service has determined that the person nominated for this job is to satisfy a pre‑employment check before taking up the appointment, on promotion or transfer. The following checks are to be conducted:

• Conviction checks in the following areas:
• crimes of violence
• sex related offences
• serious drug offences
• crimes involving dishonesty
• Disciplinary action in previous employment check.
