About you: 

You are a qualified Registered Nurse with demonstrated ability to act as an advocate for nursing in a broad context and across disciplines using positive role modelling and adherence to corporate values, ability to lead with confidence and integrity and competence to deliver a high-quality service with the primary focus on achievement of organisational goals whilst supporting patient centred care, creation of partnership and improving patient outcomes.

Your opportunity: 

The Nurse Navigator (Navigator Lead) is a Registered Nurse with advanced clinical knowledge who is accountable at an advanced practice level for the coordination of complex patient care, who supports the Nurse/Midwife Navigators in providing individual assistance to patients with a chronic/complex disease profile and their families and care givers to help overcome healthcare system barriers.

Your role: 

  • Provide effective and efficient coordination of the navigator service in collaboration with all stakeholders and provide patient centred care coordination across the patient journey by enhancing systems integration and reducing fragmentation and barriers.
  • Actively contribute to the development and review of practices, resources, policies, and procedures that support the management of individuals with complex issues.
  • Establish supportive and collaborative relationships with and between all clinical and community coordinators.
  • Participate in the data collection monitoring and evaluation process of the Navigation model by maintaining accurate records and statistics to report on relevant key performance indicators.
  • Provide effective and efficient management of service in collaboration with the Nursing Director/Director of Nursing, clinical and non-clinical staff to facilitate the provision of quality evidence-based care.
