About the role

This position will work in collaboration with other community clinicians to reduce emergency department presentations, readmissions and overall lengths of stay for the identified consumer cohort.

Intensive Support Clinician – Senior Health Practitioner $124,435 - $133,901 per annum

Clinical Nurse Consultant Intensive Support Clinician – $132,562 - $143,924 per annum
