About the role

This position is responsible for facilitating the implementation of the Eat Walk Engage program on selected ward/s. This is a geriatric inpatient initiative supported by the Healthcare Improvement Unit and an expert facilitation team at West Moreton Health (WMH). Eat Walk Engage (EWE) is a patient-centred care program designed to prevent delirium and promote high quality acute care for older hospital inpatients. The program uses an evidence-based implementation framework to embed sustainable change in practice.

The purpose of this role is to lead the implementation of Eat Walk Engage on the wards  7B and 5F  by facilitating the implementation of initiatives that will prevent delirium for older inpatients by providing high level expertise in clinical service delivery; project management and project coordination; education and training; and stakeholder engagement, liaison and negotiation at all levels

About you

Our ideal candidate will be someone who can demonstrate the following:

  • High level of clinical knowledge, experience and skills in the prevention of delirium and the delivery of older-friendly healthcare principles and practices.
  • Knowledge, skills and expertise in quality improvement methodology applicable to the Eat Walk Engage program.
  • Ability to establish and maintain effective professional relationships with consumers, colleagues and stakeholders to inform and influence clinical outcomes and behaviour change.
  • Ability to disseminate professional development learning to clinical professionals.
  • Ability to monitor and report on clinical work practices and outcomes, as well as initiate, plan and evaluate local service improvement initiatives.
