Title: Licensed Social Worker - LCSW-Crisis & Triage ManagementLocation: Honolulu, HI, USWork Schedule: Hybrid (2 days in office, 3 days from home after new hire orientation period)Job Type: PermanentSalary: USD $98,322 $98,322 / yrJob Description: This role offers a $10,000 Sign-On Bonus and a Flexible, Work from Home Hybrid Work Schedule!Job Summary: As a Licensed Social Worker specializing in Crisis & Triage Management, you will provide ambulatory behavioral health services, including evaluation, diagnosis, treatment planning, case management, and psychotherapeutic intervention for individuals, families, groups, and/or Intensive Outpatient Programs.Essential Responsibilities:Assist with administration, development, implementation, and evaluation of social work programs and policies.Provide telephonic triage and assessment for members requesting behavioral health services.Offer telephonic crisis management for members seeking behavioral health services.Coordinate admissions and discharges for members requiring psychiatric admissions.Conduct telephonic follow-up for post-hospital and ER discharges.Conduct clinical social work consultation, diagnosis, risk assessments, and crisis interventions.Provide psychotherapy and counseling utilizing evidence-based interventions and modalities.Evaluate patient psychological and mental status using the latest Diagnostic and Statistical Manual of Mental Disorders (DSM).Assess urgent or high-risk clinical situations and make appropriate interventions.Perform case management, coordination, and monitoring of social work service plans.Assist patients in enhancing or restoring their capacities for personal and social functioning.Collaborate and consult with interdisciplinary clinical team members to support assessments and decision-making.Maintain high levels of clinical expertise and adhere to professional and clinical guidelines.Qualifications: Basic Qualifications:Masters Degree in social work from an accredited college or university, passing score on the clinical level national board examination, and supervised clinical social work training as required by Hawaii Revised Statutes (HRS) 467E-7.Licensed Clinical Social Worker (Hawaii).National Provider Identifier required at hire.Basic Life Support required at hire from American Heart Association.Preferred Qualifications:Demonstrated knowledge and skill in documentation according to national accreditation guidelines.Experience in providing behavioral health services in a primary care setting.Experience in providing services to patients with addictions issues.Experience in providing Consultation/liaison Services to emergency departments.Experience in providing evidence-based treatment including conducting individual and group therapy, intensive outpatient treatment, and/or partial hospitalization programs for children and/or adults, including people with serious/chronic mental illness.Experience in writing treatment plans.Ability to work over the phone in highly stressful situations and with interruptions.Non-Negotiable Requirements:Masters Degree in social work from an accredited college or university, passing score on the clinical level national board examination, and supervised clinical social work training, as required by Hawaii Revised Statutes (HRS) 467E-7.Licensed Clinical Social Worker (Hawaii).National Provider Identifier required at hire.Basic Life Support required at hire from American Heart Association.
