About the Role

The purpose of this role is to deliver senior social work services within Mental Health Hospital in the Home (MH-HITH) team. The Mental Health Hospital in The Home program (MH-HITH) will provide hospital substitution care, providing inpatient assessment and treatment services outside the hospital setting for people presenting/experiencing with acute episode of mental illness. The objective of the program is to provide consumers with individualised assessment and treatment alongside a supportive framework for the family/carers in their own home to manage an acute mental health issue that otherwise required inpatient care.
