The common objective of the Pharmacy Service is that patients derive maximum benefit from medications whilst being exposed to minimum risk. This position has components of research and education both to multidisciplinary teams and to pharmacy staff, and is required to identify, develop, implement and maintain medication safety systems within the area of work.  As with all HP4 pharmacy positions, participation in the weekend roster and on-call roster within Cancer Care services at the Sunshine Coast University Hospital and other sites is also an expectation. The position may be expected to participate in extended hours service both on week days and weekends at any of the SCHHS sites.

Key responsibilities include:

  • Exercise independent clinical judgement in obtaining accurate patient medication history, comprehensive medication reviews, patient education and counselling clinical verification of CHARM orders and formulation of pharmaceutical care plans utilising high level knowledge and clinical skills, including the provision of advice to medical, nursing, pharmacy staff and other health care professionals.
  • Apply professional judgement to a complex pharmacy caseload for cancer care patients by undertaking a reconciliation of medications including the facilitation/dispensing of medication supply, management of side effects and drug interactions, and follow up phone calls post initiation of therapy. Communicate and liaise as required with the patient's, their carer/support persons if required, general practitioner, community pharmacy, care co-ordinators and case managers.
  • Work with the multidisciplinary team within cancer care services to ensure targets for activity based funding are met.

Remuneration value up to $152 776 p.a., comprising salary between $124 435 - $133 901 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP4) (temporary position available for up to 12 months).
