As a Care Coordinator, you will assess anddetermine patient care needs and eligibility, provide access and referrals tocommunity services, and engage with patients, caregivers and other health carepractitioners.

Whether you work in our office, in the community, ora health care facility – you will play a lead role in providing connected,accessible, patient-centred care – and be supported by our collaborative teamthat includes over 8,000 regulated health care and other professionals.

What will you do?

· In collaboration with patients and their families, assess careneeds, determine eligibility for services, and develop individual care andservice plans

· Linkpatients with service providers

· Coordinate and monitor care plan delivery

· Establish a helping relationship with patients and their families

· Balance patient needs and choices with available resources,ensuring patients’ values and preferences are respected

What must youhave?

  • Membership, in good standing, with theapplicable regulatory body:

o College of Nurses of Ontario

  • Recentexperience in community health or a related field
  • Knowledge ofthe health care delivery system and community resources
  • Excellentinterpersonal, communication, assessment, problem-solving, anddecision-making skills
  • Effectivetime management, prioritization and organizational skills, with theability to work independently and co-operatively in a busymultidisciplinary environment
  • Establishedability to accurately complete required documentation, reports and forms
  • A validdriver’s licence and access to a reliable vehicle
  • Advancedoral and written proficiency in English and in French is essential.
  • We have a mandatory COVID-19vaccination policy. As a condition of employment, all employees arerequired to submit proof of COVID-19 vaccination status prior to startdate.

What wouldgive you the edge?

· Experienceworking with diverse patient groups, e.g., multicultural, homeless, palliative,acquired brain injury (ABI), mental health, geriatrics, pediatrics

· Casemanagement experience or recent related community experience

· Abilityto speak French or another second language

French Proficiency: French required

Salary: To be determined

Terms of Employment: Temporary, Full-Time
