Enrolled Nurse/ Care Coordinator – Gold Coast

We’re currently looking for a full time EN - Care Coordinator to join our client coordination team servicing our Queensland – Gold Coast Region.

Key Responsibilities of a Care Coordinator:

  • Provide service coordination, assessment, care planning and review, including the clinical component of care plan under the guidance of the clinical team (e.g. Home Care Packages to ensure effective co-ordination and delivery of care services to clients.
  • Ensure all reporting and documentation requirements are met, including completing and maintaining timely, accurate and efficient administrative and communication systems.
  • Provide practical assistance relating to meeting quality and audit requirements.
  • Manage a prescribed caseload including assessment and care planning to remain within budgetary constraints of the service users designated program(s).

This role will report to the Clinical Care Manager

What do we need from you?

  • Minimum Diploma of Nursing or working towards.
  • Current AHPRA Registration as an Enrolled Nurse or Registered Nurse – Essential
  • Demonstrated understanding and knowledge of the needs of the clients from funding programs (e.g. Home Care Packages)
  • Familiarity with the Aged Care Act
  • Excellent computer skills with demonstrated experience in MS Office and Visual Care (Highly regarded)
  • Demonstrated commitment and experience in providing high quality customer service with excellent communication skills (written and verbal)
  • Excellent organisational and time management skills and ability to work within limited time frames
  • Subscribe to a culture of teamwork, caring, and quality service
  • National Police Check (Valid within the last 12 months)
  • Evidence of COVID-19 vaccinations
  • Current First Aid & CPR
