Our Melbourne North office has recently started and looking for an experienced and dedicated clinical care coordinator to support our growth.

Successful candidate will preferably have a strong background in community aged / disability care as . Care manager role will require Coordination and delivery including ongoing client assessments and development of care plans that deliver flexible and personalized in home care support.

Bachelor of nursing 

Current APHRA Registration 

Current First Aid & CPR

Drivers License 

National police check 

Experience in providing direct client care and ensuring safety .

Good Understanding of the Govt funding environment for aged acre and NDIS (not compulsory but preferable).

Good understanding of Hospital discharge environment .

Good understanding of health assessments and developing care plans to meet clients care needs.

Ability to multi tasking , time management.

Excellent verbal and written communication skills.

Intermediate to advanced computer skills.
