About your opportunity:

Permanent full-time

This is an Identified Position - Under s25 of the Anti-Discrimination Act 1991, there is a genuine occupational requirement for the incumbent to be indigenous to the Aboriginal or Torres Strait Islander Community.

The primary purpose of this position is to facilitate effective, coordinated and culturally capable health care for First Nation patients, their families, and carers by following up in the community and linking in with appropriate services depending on patient needs. This position will be managed by the Nurse Navigator and follow direction from the Senior Aboriginal and Torres Strait Islander Health Worker whilst providing leadership and guidance to other Health Workers in the team who are employed at a lower level.

The service will be provided through a First Nation Nurse Navigator and Aboriginal & Torres Strait Islander Health Worker led model. The service aims to strengthen partnerships with local and regional General Practitioners, as well as primary and community care networks to improve outcomes for patients and deliver effective integrated care. 

Generous remuneration package value up to $97,236 p.a. comprising:

  • Salary range of $3078.20 to $3266.60 p.f. (HW5)
  • Annual leave with relevant leave loading
  • 12.75% employer contribution to superannuation.

Requirements:

  • Under s25 of the Anti-Discrimination Act 1991, there is a genuine occupational requirement for the incumbent to be indigenous to the Aboriginal or Torres Strait Islander Community.
  • Proof of qualification (Diploma in Aboriginal and Torres Strait Islander Primary Health Care Practice, registration with the Australian Health Practitioner Regulation Agency (AHPRA) and possession of a current practising certificate.
  • This is a Vaccine Preventable Diseases (VPD) risk role: Hepatitis A/B, Measles, Mumps, Rubella, Varicella, and Pertussis.
  • Possession of class C drivers' licence.
  • Pre-employment checks will be conducted.
