Diabetes specialist nurse
Salary scale dependent on experience – position 20-25 hours
Are you passionate about diabetes management and patient care? We are a supportive, friendly and well-organised practice with over 15,000 patients looking for a motivated, dynamic and enthusiastic practice nurse with expertise in diabetes management to join our multi disciplinary team. The successful candidate will work with our clinical team of GP’s, ANPs, Practice Nurses, pharmacists, physiotherapists and an experienced team of administration staff.
We are a welcoming, forward thinking practice and proud to provide primary care from a purpose built surgery situated 7 miles west of Aberdeen. We benefit from modern attractive, purpose-built premises including a staff gym with easy access to the practice via AWPR.
Key Responsibilities:
· Conduct comprehensive assessments and develop personalized care plans for patients with diabetes.
· Educate patients on self-management techniques, diet, exercise, and glucose monitoring.
· Pharmacological management of diabetes including initiation of and ongoing management of patients on insulin.
· Collaborate closely with our multidisciplinary team to ensure holistic patient care.
· Maintain accurate electronic health records and participate in quality improvement initiatives.
Qualifications:
· Registered Nurse with a valid license to practice.
· Experience in diabetes management and a strong understanding of evidence-based guidelines.
· Excellent communication skills and the ability to empathize with patients.
· Proficiency in using electronic health record systems.
