IHP is looking to add an APP to our team as we expand our services to new facilities. IHP is a local, physician-owned, provider management organization that offers inpatient care in short term and long term hospitals, rehab facilities, post-acute facilities and SNFs.

Basic Job Details:

• Looking for compassionate, caring and talented APP to cater to the medical needs of seniors
• Monday-Friday, 8a-5p or 9a-6P
• Call One in 3 weeks/weekends per month
• 14-16 patients per day
• Take care of the patient in post-acute care and long-term facilities
• EMR(PCC) utilization/notes

Qualification:

• Board-certified APP, NP or PA
• 1 year Experience caring for the geriatric population and post acute care a plus
