Job Description

MAIN DUTIES AND RESPONSIBILITIES:


• Assess the shared information on patients’ need with the healthcare team for sustainable integrated care delivery.
Recommend and innovate the provision of integrated care by eliminating barriers in accessing hospital services in
collaboration with the involved sections within the Clinical Strategy Department.
• Communicate with various healthcare specialties and document the complete patients’ treatment plan. Review the
integrated care in place and recommend any changes in relevant policies with well-defined guidelines for smooth
processes of medical care requirements.
• Participate in multidisciplinary meetings pertaining to the integrated care and the patients’ need in relation to
hospital services, as required. Participate in developing and implementing the required procedures, protocols,
clinical practice guidelines for concurrent use of integrated care program.
• Collate data information related to the implementation of integrated care program with emphasis on high volume,
high risks, high cost, and length of stay diagnosis/procedure. Inspect the patient care activities and report
information variances and gaps as well as risks within the integrated care system.
• Prepare recommendation on newly developed systems that will facilitate more effectively and efficiently the
patient journey through integrated care. Ensure that integrated care systems are meeting the patients’ needs for
healthcare and social services.
• Participate in coordinating with the medical departments, medical team and other involved healthcare
practitioners to continually assess the information on patients’ health conditions and liaise the appropriate
education and self-management support as applicable.
• Ensure that all work is performed in line with policies, safety standards, regulations and accrediting bodies.
• Perform other job related duties.

Education: Bachelor Degree in any Health Administration/Sciences or Healthcare profession – Essential; or Bachelor Degree in non-Healthcare Profession with Master’s Degree in Quality and Health Management/Head Administration/Business Administration – Essential

Certificate in Healthcare Quality Management or equivalent – Essential

Work Experience: SCDP - Saudi Career development (Training program) 

English Language: Level – I – Essential
