Position Summary

Do you have a passion for nursing? We have the position for you! We are seeking a motivated Registered Nurse with a dedication for delivering compassionate patient care to join our team at Brunswick.

In accordance with Northern Health’s vision of an idealized system of services, built on a foundation of Primary Care, where people and their families receive primary care services in Primary Care Homes supported by interprofessional teams. The Tele-Care Nurse (TCN) practices in accordance with the standards of professional practice and code of ethics as outlined by the BC College of Nurses and Midwives BCCNM). The TCN competently applies the nursing process of assessment, planning, implementing, evaluating and documenting care to clients. Using a variety of specialized communications and information technologies, Tele-Care nursing activities focus upon enhancing clients’ ability to improve their health and make sound health care decisions.

Reporting to the Clinical Team Lead, the TCN is responsible for providing safe, competent and ethical Tele-Care nursing in accordance with NH mission, strategic plan, legislative requirements, operational policies and clinical guidelines.

The TCN conducts patient registration, assessment, crisis intervention, counselling, education, triage and connections to Providers and Interprofessional teams through virtual tools. They utilize care protocols, standards and guidelines in arriving at their decisions.

Qualifications

• Graduation from an approved School of Nursing with current practicing registration as an RN with the BC College of Nurses and Midwives (BCCNM).
• Minimum three (3) years recent, related clinical nursing experience in nursing and/or chronic disease management settings, including experience developing and providing formal education to patients, families, the general public, or an equivalent combination of training and experience.

• Human Caring and Relationship Centered Practice – Ability to promote client-focused care that demonstrates care for and with clients and significant others, sensitive to diverse cultures and preferences, client advocacy and social justice concerns.
• Communication – Demonstrated ability to communicate effectively with the clients, families, the public, medical staff and the members of the interdisciplinary team using verbal, written and computer communication means.
• Critical Thinking – Demonstrated ability to integrate and evaluate pertinent data (from multiple sources) to problem-solve effectively.
• Innovation: Demonstrated ability to challenge conventional practices; adapt established methods for new uses; pursue ongoing system improvement; and evaluate new technology as potential solutions to existing problems.
• Tolerance of Ambiguity: Able to deal with unresolved situations, frequent change, delays or unexpected events.
• Teaching – Ability to transmit information intended to instruct clients and others about topics essential to health care and well-being.
• Assessment and Intervention – Demonstrated ability to complete initial and ongoing client assessments (clinical and diagnostic reasoning) and provide nursing care through appropriate/ prescribed technical, therapeutic, safety type interventions.
• Ability to operate related equipment including proven ability to utilize computer technology.
• Physical ability to perform the duties of the position.
