This role will be responsible for the coordination and management of integrated health services within the Doomadgee ( https://www.doomadgee.qld.gov.au/ ) region through the Community Health Centre in collaboration with the Doomadgee Hospital services and other external partners.

Effectively manage, coordinate and deliver services to clients/consumers through discharge planning, referrals, coordination of community based care, delivery of health education, screening and promotion programs, while delegating appropriately to health workers to achieve a safe effective outcome.

PERMANENT FULL-TIME ROLE.

Base salary range between $132,562 and $143,924 p.a. (Nurse Grade 7)

Requirements:

• Proof of qualification, registration with AHPRA and a current practising certificate are mandatory requirements.
• This is a Vaccine Preventable Diseases (VPD) risk role and may require evidence of vaccination or immunity to: Hepatitis B, Measles, Mumps, Rubella, Varicella, Pertussis, Tuberculosis and COVID-19.
• A 6-month period of probation may apply.
