Our office is in Mitchell ACT. We service the ACT and Surrounding NSW areas. 

We are seeking an experienced Registered Nurse to take on the role of Care Manager.

Salary Range: $85,000 - $110,000. Plus Super.

The Care Manager (Registered Nurse) will oversee the care and coordination of all allocated Client/Participant related activities within Right At Home ACT & Queanbeyan, ensuring that a high quality of consumer directed care is delivered to all Client/Participants within the statutory requirements. Care Management is a ‘mission critical’ responsibility for the delivery of care to Right at Home's Client/Participants. This includes the recruitment, training and quality control of carers and the related systems and processes. The Care Manager will support the Care Coordinator in developing care plans and handling complex client situations. 
In addition to Care Management this role will help develop relationships with hospitals, clinics, and care facilities with a view to delivering care services to meet the market.

About You

To be considered, you will need

  • Experienced RN (tertiary qualification)
  • Current AHPRA registration
  • Sound knowledge regarding Aged Care Sector and NDIS (My Aged Care, Home Care Packages, Funding, Care Planning, Government Guidelines, up to date with sector changes etc)
  • Ability to provide management and leadership to Care Coordinators, Personal Care Workers and Companion Carers. 
  • An understanding of the impact of complex care needs and a demonstrated ability to apply high level of clinical knowledge (including palliative care, dementia care, wound care).
  • Current National Police Check (Mandatory)
  • WWVP Card (Mandatory)
  • Drivers Licence and Comprehensively Insured Private Vehicle (Mandatory)
  • Current First Aid and CPR Certificates (Mandatory)
  • NDIS Clearance
  • Smart Phone
  • A positive attitude and good communication skills and an approachable manner
  • Willingness to learn, develop and take on new challenges
  • Willingness to complete Right at Home Mandatory Training Modules
  • Willingness to complete Statutory Declaration 
  • A can-do attitude that is willing to help us grow and develop our client base and referral network
