About us:

The Sub Acute Chronic Care Rehabilitation (SACCR) team provides interdisciplinary rehabilitative care to clients with a range of complex chronic diseases. These services also work to address unplanned medical admissions and support early discharge by enhancing the quality and availability of community based proactive collaborative care.

This service uses the interdisciplinary client centred model as described in the SACCR service profile, with student placement and clinical education embedded in this core business.

About the role:

A Nurse Practitioner (NP) – Chronic Disease is a Registered Nurse who:

  • Functions autonomously and collaboratively in an advanced and expanded clinical
  • Uses expanded skills and knowledge in assessment planning and implementation, diagnosis and evaluation of nursing care
  • Works closely with the team to establish and manage the CQ Respiratory Rapid Access Service which will form part of the hospital avoidance strategies available across Central Queensland (CQ).
  • Assesses and manages recipients of healthcare services using advanced nursing knowledge and skills which may include however not limited to:
    • Direct referral of recipients of healthcare services to other healthcare
    • Prescribing medications and,
    • Ordering and interpreting results of diagnostic

About you:

To be successful in this role you must have:

  • Mandatory qualification/s: Current nursing registration through the Australian Health Practitioner Regulation Agency (AHPRA).
  • Nurse Practitioner Masters level qualification and NMBA endorsement to practice as a Nurse Practitioner is
  • The successful applicant will be required to undergo the CQHHS Nursing and Midwifery and Health Practitioner Credentialing and Defining Scope of Clinical Practice Sub Committee Nurse requirements as a Nurse Practitioner according to the CQHHS
  • Demonstrate evidence of provision and of coordination of comprehensive contemporary person-centred care through advanced practice
  • Demonstrate advanced and expanded clinical knowledge and skills in the specialty field of chronic disease
  • Experience in developing structures, systems, and processes to ensure quality assurance and efficient service
  • Experience in collecting and analysing service data; reporting variances and benchmarking to align practice service with best

Salary information:

Total remuneration value of up to $177,797p.a. is comprised of:

  • A hourly rate between $75.27 - $78.59 p.h.
  • 12.75% employer superannuation contribution
  • 17.5% leave loading
