What You'll be Doing
The Out of Home Care (OOHC) Health Pathway Program is a key initiative arising out of the Keep Them Safe: A Shared Approach to Child Wellbeing 2009-2014, which is the NSW Government Response to the Report of the Special Commission of Inquiry into Child Protection Services in NSW.
The OOHC HPP works across the broad range of health services as well as with interagency service partners to improve the health outcomes of children and young people in care.
The OOHC Clinical Nurse Specialist facilitates and implements health assessments and Health Management Plans for children and young people entering Out of Home Care (OOHC) within South Western Sydney Local Health District (SWSLHD). The position will work within a multidisciplinary and multi-agency approach with key stakeholders both within SWSLHD, and externally with key government and non-government partners including Department of Communities and Justice (DCJ), Department of Education & Communities, General Practitioners, private health care providers, non-Government organisations, as well as children and young people in OOHC and their carers. The position has clinical, liaison, implementation and monitoring functions.
