About the role
The purpose of this role is to apply Occupational Therapy skills and knowledge in the assessment and treatment of clients in the local population in order to promote, maintain and improve their health, functional ability and quality of life. The Post Acute Care Service (PACS) provides a post-acute hospital avoidance response to people who are discharged from hospital or who present to emergency departments and are not admitted or referred by GP or QAS and do not require hospital admission. Short term multi-disciplinary interventions are provided to address immediate care needs. Services are provided in-home or clinic based. PACS operates seven days a week including public holidays with extended opening hours – 0730 hours to 1800 hours. Further extension of operating hours may occur based on client and hospital and health service need in the future.
