Integrated Pharmacist

Alexandra Hospital, Ingersoll and Tillsonburg District Memorial Hospital is actively seeking applicants for a Part Time, Integrated Pharmacist.

Key Responsibilities

Systems Management

  • In partnership with clinical staff and physicians, ensure security and safe storage of narcotics, controlled and other drug supplies throughout the hospital.
  • Instruct the members of the clinical team on best practices and policies for all aspects of the medication use cycle with a focus on prescribing, administration and documentation of medication.
  • Organize, direct and administer the daily activities of the service when the Pharmacy Manager is absent.
  • Develop new policies and procedures to support the safe and effective utilization of medications in the hospital and develop methods to track adherence.
  • Select, purchase, acquire, manufacture if necessary, distribute, and provide safe use and storage of pharmaceutical products in accordance with NAPRA standards.
  • Participate in the short term and long term strategic planning of the hospitals and departments goals and objectives. Direct efforts as required to meet overall hospital objectives.

Professional Accountability

  • Ensure Pharmacy practice is ethical, legal and professional.
  • Follow the Government of Canada's mandatory requirements for hospitals in reporting serious adverse drug reactions.

Qualifications Required

  • Graduate from an accredited post-secondary Pharmacy Program (i.e. BScPhm, M. ScPhm, PharmD).
  • Member in good standing with the Ontario College of Pharmacists (OCP) and Part A License with the OCP. Preference will be given to persons with experience of a residency in Hospital Pharmacy.
  • Strong team player with the ability to perform duties independently and professionally.
  • Excellent written and verbal communication skills.
  • Strong interpersonal skills and the ability to communicate effectively with a diverse range of individuals.
  • Flexibility to meet operational requirements, including hours of work or on-call situations.
  • Prioritizes and responds effectively to changing work demands.
  • Shows initiative and identifies key issues, and takes logical responsible approach to problem solving, trouble shooting and conflict resolution.
  • Commitment to ongoing education.
  • Willingness to adapt to a changing health care environment.

Other pertinent conditions:

  • Salary ($48.21 per hour to $64.50 per hour)
