The Position 
Our innovative service model focuses on delivering the best clinical outcomes for clients. As an experienced Dietitian with Empower, you will have the opportunity to work with a variety of patients across home care in the Northern and Southern suburbs of Perth. 
This role is a 12-Month Maternity Leave Cover. 
You will:
  • Complete comprehensive nutritional assessments for residents and for clients in their own home through home care packages/NDIS.
  • Develop personalized nutrition plans and goals
  • Support the dietary requirements of individuals with difficulty swallowing and chewing, bowel and intestinal issues and other complex issues.
  • Provide weight management, poor appetite and dysphagia interventions
  • Manage food intolerances and allergies
  • Provide education on diabetes management
  • Educate clients and their families to optimise their ability to live healthy lives within the community
  • Work holistically with other allied health professionals to provide a multidisciplinary care plan
  • Liaise with service providers to ensure effective communication
  • Ensure caseload and documentation are timely and completed at a high standard 
You will be provided with support from our home care team who will schedule these appointments and plan an efficient route.

We are looking for someone that is passionate about making a difference in the lives of our people. You will have an enthusiastic and proactive attitude, be outcome focused and dedicated to delivering an exceptional service to our clients.
You must have: 
  • Bachelor of Nutrition and Dietetics (or equivalent) 
  • Experience working within home care, aged care and NDIS settings 
  • Membership with Dietitians Association of Australia with Accredited Practicing Dietitian status
  • Police Check Clearance 
  • Excellent verbal and written communication skills
  • Strong time-management skills and attention to detail
  • Ability to work independently and as part of a team
