About the role:

The purpose of this role is to provide clinical practice in an integrated model of care called Hospital in the Home (HITH) which is designed to prevent hospital admissions and ED avoidance and support shorter length of inpatient stay. This service is delivered across the community and acute interface as a 7-day (8am-8pm) service and incorporates medical/clinical governance, patient and staff safety, and operates within the parameters of the SLHD.

The successful applicant is expected to be an experienced nurse with confidence in acute care and/or wound care or working towards the same. They will be required to work independently in the community and clinic setting providing acute care with an ability to recognise the limits of their scope and escalate appropriately within the interdisciplinary team. The Registered Nurse Hospital in the Home delivers high quality nursing care to patients in accordance with the Nursing and Midwifery Board of Australia’s National Standards of Practice for Registered Nurses, criteria for RN as outlined in the Public Hospital Nurses and Midwives (State) Award and the Policies and Procedures of the MoH and Sydney Local Health District (SLHD).

About you:
We are seeking a skilled and energetic Registered Nurse who possess a passion for person centred care. We welcome applications from people who have at least 2 years’ experience.
